DISC assesment form

The project is to make an existing DISC assessment form work and output to email. Original form was using a Server, no longer available, and no longer works. A DISC assessment consists of 25 questions. Each question consists of 4 adjectives, Candidates select one value is marked "M" for most likely to apply to the candidate, one "L" least likely to apply and the other 2 are blank. The drop down code for all the questions exists. my client owns the code. The for outputs to formtoemail.com. Example: gentle, persuasive, humble, original. candidate selects one of the four "M", one as "L" leaving 2 blank. At the end the forms is submitted and the answers form a matrix that can be used t assess personality type. There are the areas that are needed. 1. The original page was .asp, it must be .html 2. We need an error check routine so that each question can only contain, one "M", one "L", and two blank answers. It must check line and complete form and highlight error. 3.
