I propose a straightforward approach:
1) Data configuration module
- Here you can configure information items as a data dictionary (types of procedures, tests, rest results, ranges, patient collected information and privacy levels, etc). You can virtually add anything as the dictionary is divided into sections which you also define
- The menus are also defined here
2) User maintenance
- You define system users, including name, password, contact information
3) Patient management
- First part includes identification (code, name, contact information)
- Second part includes medical information maintenance
4) Data export module
- Only authorized users will be able to extract information and export it in different formats
