I need an intro for my company similar to this one

It shouldn't be exactly the same but I need 3d looking icons. A voiceover and music will be provided. The duration should be between 40 to 45 secs and should be in QuickTime format 720p.
I want it done in 5 days max. So only bidders who think can manage this in 5 days should bid. One more thing, I would want to see your portfolio or similar motion graphics work.
Finally I want someone who can do a neat job in the time mentioned above.
