If you mean attaching document I have these ideas : 1. Make an hyperlink. or 2. Attached like OLE access 3. Print Screen automatic and Paste into form. But if you need every new person create form will make a new document (filled) and the blank document stay, I think Macro VBA is suitable for that. I'm waiting your response, thank you.

I will create three forms in PHP. One form as you have indicated in which first person will fill all details and press submit. It will show the second form and second person will fill second form detail and press submit. Then it will show third form to be filled by third person and when he presses submit, the PHP will create an Excel sheet with all three form details. Krishnan
