Create database/form functionality on website

We are in the process of building this site: [url removed, login to view]~wwwmicha/
As part of the build we need to move ahead with the form/database functionality. Currently what is on the site is just dummy content.
1. The first task will be to edit the homepage form on the left of the screen. It will be changed to "Job Seekers - Submit Your Details". Attached is a spreadsheet that refers to the form details and process of the form.
2. On the subpage, currently there is an item that says "this weeks top jobs". This will be replaced with "Company and vacancy registration" database which is attached.
The coder may choose to edit the page templates because currently there is no library in place to update all pages.
