Convert my word file to eBook (MOBI, EPUB, etc.) so it appears PERFECTLY as an eBook, preserving the formatting

I need someone to PERFECTLY convert my word document (which I have diligently formatted) to the various eBook formats (EPUB, MOBI, etc.), so I can upload them to Amazon for Kindle, and Itunes. I originally gave this project to another freelancer, but there were too many issues, including: - page numbers on TOC not corresponding with the actual page numbers - some paragraphs not having a line separation - some pages showing a heading title, only (instead of continuing on with more text) - the loss of the important horizontal lines at the end of chapters - the horizontal line at the end of a chapter showing on a page by itself (need to use 'keep text together' to ensure this doesn't happen) - the footnotes show at the end taking up a full page, each So please, only bid if you are proficient in this service. I need it to be perfect. It shouldn't take too long, but will be a process of not only converting the file, but ensuring that all the formatting in the word document is perfect, and preserved by the conversion.
