Access Database to Track Patients in Medical Practice

We need someone to write a program (? MS Access 2007) that can be networked with our Windows PCs in a small medical practice. Our goal is to have a means to track the referral sources of our patients which could include referring physicians, Internet, insurance company or other sources. The data that we wish to compile include the following: patient name, town, state, referral source (with drop down menus), age, name of physician seeing the patient,data first revisit appointment, data second revisit appointment, treatment start date, type of treatment (drop down menu), outcome of treatment (drop down list). Our goal is to track which patients fail to follow-up with our group of doctors and why. This should be a drop down list under why they didn't return to indicate the reason which may include: moved to another area, consulted with another physician, no insurance, financial issues, etc. If the patient is entered as a new patient in the database and does a return within a prescribed time frame, pop-up reminder or e-mail should be generated for the program to alert us. The UI should be simple to use for our non-computer literate physicians. We should also have the ability to run reports on the data entered in the program.
