Information Required:

a. Company Name

b. Key Personnel – Capture each of Chairman, CEO, Managing Director, General Manager, Director, and CFO. List details of each key personnel on individual row. Separate columns for each of the Firstname and Surname. Capital letters for both Firstname and Surname.

c. Key Personnel title. (Mr / Mrs / Ms / Dr)

d. ABN/ACN – Capture ABN in the first instance. If not provided capture ACN.

e. Address – Capture Physical address in the first instance, if not provide use postal address. Use separate columns for each of:
