Database Driven Web

We are building a Database Driven Video website. We need a programmer to take the input form attached and create 4 web pages. 1. Create a web based input form from the (Video Form PDF sample) 2. Create an interface to select the form for later editing 3. Create a Library or catalog page that is filter driven by the fields in the form. These filters will be in the background and set by us to allow specific people access to specific subject matter. The user may search the filtered information to find his or her video. The database will contain thumbnail images of the videos (Sample Page 1 PDF) 4. A single database driven player page that loads the videos, the download materials that would go along with the videos, and automatically places the subject matter and video name in the “Ask a Question” input form.
