Adobe form populated w e-signatures

I have an Adobe form (merchant application for credit card processing). I had the ability to complete the form along with 3 e-signatures using a 'wizard' that populated the form, requested and inserted signatures, which would then render a completed form including signatures (I LOST THIS ABILITY THROUGH MY PROVIDER). I want to recreate this same funcationality so that I can complete an application, along with signatures, creat a pdf which i can email. A wizard would be nice, but time an money are of the essence, so a long list with the fields, drop downs, and radio buttons would also work. Thanks for your interest!
