Designs needed for signup forms

We are making some lightbox optin/signup forms. What exactly is an optin form? Well, it's a form that asks a user for their name and email address. Our concept is a little different though. **V. IMP** --> The forms will be overlaid on background images. We have the bg images (around 40) and the form will be superimposed on top of these backgrounds, left, centre or right wherever it looks good. The button design, the frame everything should look good so that it suits the background image. Your bid is for creating forms for overlaying on existing images. Now even if there are 40 odd images you don't necessarily have to create 40 forms and button designs, you can repeat the same design if it suits another background but yes we cannot have the same style for every background. Some may have semi transparent frame while some may have completely opaque, the quality should be of the popudomination and pippity standards perhaps even better. Just to give you an idea I have made one myself, now it definitely does not look great that's why I want a professional to be making it, YOU. Tip: Do you see that background image with beer in it, you can probably try to show off your creativity by making your form design on it.
