We need you to make a missed time punch for hourly employees with our logo on it - we have two different logos so we will have you send two pdfs at the end of this.
This form will let you make changes to your time card, use sick pay - which has a checkbox and also let you request time off
Both the employee and the supervisor must have signature areas.
Design doesn't need to be so fancy, just easy to read the font and fill out the area. We have attached a sample with the kind of content we need as well.

You will create two PDF files that you will design and add all these areas in an easy to fill out format/form
